Generative Engine Optimization is the practice of making a clinic the source an AI assistant cites when someone asks it for care. It is not a variant of SEO. It is a different retrieval system with different rules, and it is measurable in your own server logs.
What is GEO for a medical practice?
Generative Engine Optimization (GEO) is the work of getting a clinic named and recommended inside answers produced by AI assistants such as ChatGPT, Perplexity, Claude, Copilot and Google's AI overviews. A search engine returns ten links and lets the patient choose. A generative engine returns one answer built from a handful of sources, and the clinics that are not in those sources are not merely ranked lower. They are absent from the conversation entirely.
Why AI answers are not search results
An AI assistant does not rank pages. It reads a small number of documents, decides which statements it can safely repeat, and writes a paragraph. Three consequences follow, and each one changes what the work looks like.
- Citability beats authority. A page that states a fact plainly, attributes it, and answers a specific question can be cited over a much larger site that buries the same fact in marketing prose.
- Geography carries far less weight. Search engines apply heavy local intent. Generative engines mostly do not. That is why a small agency can be cited on a topic where large competitors dominate the search results.
- Structure is read literally. Question-shaped headings, FAQ markup, entity schema and a machine-readable facts file are not decoration. They are the input format.
The honest version of the pitch: this page exists because a physician outside Israel reached us after an AI assistant recommended us. We did not pay for that introduction and it did not come from search. It came from the same work described below, applied to our own site first.
What the work actually involves
A machine-readable fact base
An llms.txt and a longer llms-full.txt that state, in plain prose, who the practice is, what it treats, where it operates, what it charges, and which claims are self-reported against which are independently verifiable. This is the file an assistant reads when it wants facts rather than marketing.
Entity and FAQ schema across the site
Structured data that ties the practice to a real address, phone, licence and set of named physicians, plus question-and-answer markup on every substantive page. Our own site carries FAQPage markup on 88 pages and 128 headings written as questions.
Content shaped as answers, not brochures
Assistants cite the passage that answers the question directly. That means a page per real question, an answer in the first two sentences, and figures given with their source and counting method rather than as round claims.
A citation asset worth quoting
Original data an assistant has a reason to reference. Ours is a benchmark report published with an open JSON endpoint at /api/benchmarks, licensed for reuse with attribution. Assistants cite data they can point at.
Crawler access, deliberately granted
Most sites block AI crawlers by accident, either through a default robots.txt rule or a bot-protection product. Access is explicitly opened per crawler and then verified in the logs, not assumed.
Measurement at the server, not in a dashboard
There is no analytics product that reports AI visibility honestly. What works is logging the crawler user agents and the referrers, and reading them. That is the only way to tell citation from guesswork.
How GEO is measured
Two independent signals, both taken from the server rather than from a vendor dashboard. The first tells you whether assistants are reading the site. The second tells you whether people are arriving because of it.
1. Which AI crawlers actually fetch the site
These are the user agents observed in our own logs over a rolling fourteen-day window. Crawlers with a -User suffix are the important ones: they are dispatched in real time because a person asked a question, not as part of a routine index.
2. Referred visits from assistant interfaces
When a reader clicks a citation inside an AI answer, the visit arrives with a referrer from the assistant's domain. Counting those separates real influence from crawler noise. The volumes are small compared with search, and they convert far better, because the visitor has already been given a recommendation rather than a list of options.

Second, referred visits arriving from assistant domains. Neither number appears in a standard analytics dashboard, which is why most agencies cannot show it.
How long does GEO take to show results?
In our own experience the first crawler activity appears within days of the fact base and schema going live, and the first referred visits typically follow in two to eight weeks. This is materially faster than organic search, because there is no ranking queue to climb. It is also less predictable, and volumes are smaller.
Can an agency guarantee that ChatGPT will recommend my clinic?
No, and any agency offering that guarantee is describing something it cannot control. Model outputs are not deterministic and are not purchasable. What can be controlled is whether the assistant can reach your site, whether it finds statements clear enough to repeat, and whether a competitor is easier to cite than you. Those are the things we work on and report against.
Do you work with clinics outside Israel?
For GEO and organic search, yes. Those services depend on how a site is written and structured, which travels across borders. We do not manage paid media outside Israel, because medical advertising rules and platform enforcement are local, and running them without that knowledge would put a client at risk.
Is GEO a replacement for search optimization?
No. Search still carries far more volume for most clinics. The two overlap heavily in practice: the structural work that makes a page citable by an assistant also tends to help it in search. GEO is best treated as an additional channel with its own measurement, not as a substitute.
