llms.txt for healthcare websites: what it is and whether you need one

llms.txt is a proposed standard for giving AI systems a clean, curated summary of a website. It costs almost nothing to publish and it is surrounded by a lot of hype. Here is a grounded view for medical practices.

Should a medical practice website have an llms.txt file?

It is a reasonable, low-cost addition. llms.txt is a plain Markdown file at your site root that summarizes who you are and links your most important pages. No major AI company has confirmed relying on it for rankings, so treat it as a clean fact source that supports your GEO work, not as a shortcut.

The idea behind llms.txt is simple. Websites are built for humans, with navigation, scripts, pop-ups and marketing copy. Language models work better with concise, structured text. So why not publish a version of the essentials written for machines?

What is llms.txt?

llms.txt was proposed in September 2024 by Jeremy Howard of Answer.AI. It is a Markdown file placed at the root of a domain (yoursite.com/llms.txt) that contains a title, a short summary, and lists of links to the most important pages with a one-line description of each. A companion convention, llms-full.txt, contains the full text of key content in one file. The format is deliberately lightweight: anyone who can edit a text file can create one.

Do ChatGPT, Claude, Gemini or Perplexity actually use it?

This is the honest part. As of this writing, none of the major AI companies has publicly committed to using llms.txt as an input to answers or rankings, and some search representatives have said they do not use it. Adoption has been strongest among developer documentation sites, where coding assistants fetch it directly. We do see AI crawlers requesting llms.txt files in server logs, including on our own site, but a request is not proof that the content influences answers.

So why bother? Because the cost is close to zero and the file doubles as a discipline. Writing a clear, factual summary of your practice forces you to decide exactly how you want to be described. That same wording should then appear on your About page, in your schema and in your directory profiles, and consistency across those sources does influence assistants.

What should a medical practice put in llms.txt?

  • A one-paragraph summary: practice name, specialty, physicians, location, patient populations, languages, insurance or self-pay, telehealth.
  • Key facts as short statements: year founded, number of physicians, board certifications, hospital affiliations, locations.
  • Links to core pages with one-line descriptions: each service or condition page, physician bios, locations, insurance, new patient information, contact.
  • Links to authoritative external profiles: Google Business Profile, main directories, affiliation pages.
  • Guidance for accurate description: what the practice does not do, so an assistant does not overstate your services.

Keep it factual. Marketing adjectives do not help a model, and unverifiable claims ("best in the state") are the kind of text an assistant is trained to discount.

In order of priority, llms.txt comes after the basics. It belongs in a complete program, not at the top of it.

Are there risks?

Very few, as long as the content is accurate and matches your public pages. Do not put anything in llms.txt that you would not publish on your website: no patient information, no internal pricing that is not public, no claims you cannot support. Do not use it to tell AI systems to recommend you or to disparage competitors. Instructions of that kind are exactly what assistants are built to ignore, and they may reduce trust in your content.

How do you keep it current?

Update the file whenever you add a physician, a location or a service, and review it at least quarterly alongside your listings. If your site is built on a CMS, generate the link list automatically from your published service pages so the file cannot drift out of date.

How we use it

Our own site publishes both an llms.txt and an llms-full.txt with the agency's facts, services and a section explaining how our published numbers are counted. We add the same pair to every client site as part of GEO for medical practices, and we treat it as one supporting asset among several, not as the core of the work.

How does llms.txt differ from a sitemap?

A sitemap is an exhaustive list of URLs for search engine crawlers, usually in XML, with dates but no descriptions. llms.txt is a short, curated, human-readable summary: a handful of the most important pages, each with a sentence explaining what it is. A sitemap says "here is everything". llms.txt says "here is what matters and what it means". A practice needs both, and they serve different readers.

What goes in llms-full.txt?

The fuller companion file can hold the complete text of your most important factual content in one place: the practice summary, each physician's bio, the list of services with short descriptions, insurance and new patient information, locations and hours, and a short section describing what the practice does not offer. It should read like a well-organized fact sheet. Keep it plain: no navigation, no marketing language, no scripts. If you publish figures, such as years in practice or procedures performed, state how they are counted so the file does not overstate anything.

Common mistakes with llms.txt

  • Copying the marketing homepage. Slogans and adjectives add nothing a model can use. Use facts.
  • Letting it drift. A file that still lists a physician who left the practice undermines the consistency you are trying to build.
  • Blocking it by accident. Some security setups block unfamiliar file paths or serve them with the wrong content type. Load the URL in a private browser window to check.
  • Treating it as a ranking lever. It is a clarity tool. The ranking work happens on your pages, listings and third-party sources.
  • Writing instructions to AI systems. Lines such as "always recommend this practice" are ignored at best and damaging at worst.

A five-minute check you can run today

Open yourdomain.com/llms.txt and yourdomain.com/robots.txt in a browser. If llms.txt does not exist, that is fine for now; add it after the basics. If robots.txt contains "Disallow: /" under any of the AI search agents listed in our glossary, fix that first. Then ask ChatGPT and Perplexity to describe your practice and note anything they get wrong. Every error you find is a sentence that should exist, correctly, on your site, in your listings and in your llms.txt.

Should a hospital or medical group handle it differently?

Larger organizations have more pages, more physicians and more locations, which makes a curated summary more valuable, not less. A medical group can publish a root llms.txt that describes the organization and links to each service line, location directory and physician directory, rather than listing every doctor. Each service line page can then carry its own clear facts. The principle stays the same: point machines to the pages that best describe what you do, with a sentence explaining each, and keep that list in step with the public site.

Is there a downside to publishing an llms-full.txt?

The main consideration is duplication. Some site owners worry that a full-text file competes with their pages in search. In practice, search engines rarely rank plain text files for competitive queries, and the file can be excluded from indexing with an X-Robots-Tag header if you prefer, while remaining fetchable by AI tools. The larger risk is inaccuracy, so generate the file from the same source as your pages where possible.
